Family Therapy and Systemic Practice - Clinical Supervision Groups Application Form

Please note that you are signing up for the 10-month supervision group, meeting each month for 1.5 hours.

You will be invoiced for the total amount of sessions ($850 + GST) once you have accepted/confirmed your place.  This is a closed group and your place has been reserved on this basis.

There will be no refunds if you are unable to attend one of the groups for any reason or if you leave the group prior to completing the 10 monthly sessions.
